Failure and edge-case behavior

• Security-sensitive helpers return False, 0, or empty result sets when their SQL or stored-procedure lookups find no qualifying row.
• Employee-leave create, update, and delete paths treat unexpected row counts as persistence failures and surface broken-rule or result-code failures.
• Policy-address resolution, EFT authorization, and imaging alert helpers depend on stored procedures whose branch rules live in the procedure documentation and can return empty results when no qualifying database state exists.
• Address, claim, and output-address validation helpers intentionally reject missing or inconsistent master data rather than synthesizing fallback values in the business layer.
